Placement Assistance
We carefully take the time to know the needs of the individual and family to explore next level of care if needed. We assist with placement in Independent Living, Assisted Living, Memory Care, and Group Homes. We assist with moving and set up in new environment.
Our Services
In Home Modifications
At Guided Legacy Care, we provide recommendations and modifications to promote safety and independence within their home. Our goal is to allow individuals to continue to live in their environment safely.
Private Therapy Service
We provide Physical, Occupational and Speech therapy services during placement transition to ensure loved one receives the right care.

How do I know the right care I need for my loved one?
We will guide you through every step of the process to ensure the individuals needs are met. To begin your consultation service with us, please call or email us as each individual’s care is different.
How much do your services cost?
Our consultation service and placement assistance is completely free. We do not accept money from clients in-regards to placing in a new environment. Our add on services of private therapy, moving, and in-home modifications are of cost. Cost will vary between clients.
If placement is needed, how do you know which is a right fit?
We have met and interviewed each facility that we place individuals with. We provide families with what facilities offer, cost, and add on services if needed. We do our best to make sure the placement is of the right fit.
